Desvenlafaxine
Indications/Uses
Symptomatic relief of major depressive disorder in adults.
Dosage/Direction for Use
50 mg once daily taken at approx the same time each day. Max: 100 mg daily.
Administration
May be taken with or without food: Swallow whole, do not divide/crush/chew/dissolve.
Contraindications
Hypersensitivity to desvenlafaxine or venlafaxine. Use of MAOIs (including linezolid & methylene blue) concurrently, or w/in the last 14 days. Concomitant use w/ other venlafaxine- or desvenlafaxine-containing drugs; any prescription or non-prescription medicines, including nutritional or herbal supplements.
Special Precautions
Risk of new or worsened emotional or behavioural problems; bone fracture; angle-closure glaucoma; sexual dysfunction; postpartum haemorrhage. Caution in patients w/ history of high BP; history of heart problems; existing GIT narrowing or blockage; history of fits (seizures); history of low Na blood levels; existing bleeding disorder or low platelets; recent bone fracture or existing osteoporosis or risk factors for osteoporosis; history of high cholesterol; history or family history of mania or bipolar disorder; existing kidney problems. Do not take Apo-Desvenlafaxine w/in 14 days of stopping a MAOI. Wait at least 7 days after stopping Apo-Desvenlafaxine before taking any MAOI. Wait until 24 hr after the last dose of linezolid before starting serotonergic psychiatric drugs. May affect certain lab results. May impair ability to drive or operate machinery. Pregnancy & lactation. Do not use in childn & adolescents <18 yr.
Adverse Reactions
Nausea, dry mouth, dizziness, trouble sleeping, sweating, headache, drowsiness. Heart palpitations, tinnitus, vertigo, mydriasis, blurred vision, vomiting, diarrhea, weakness, chills, feeling jittery, irritability, decreased/increased wt, increased BP, musculoskeletal stiffness, shaking, disturbance in attention, tingling sensations, taste changes, anorgasmia, anxiety, nervousness, decreased libido, abnormal dreams, delayed ejaculation (in men), erectile dysfunction (in men), yawning, rash, hot flush, decreased appetite, constipation, tiredness, drug w/drawal syndrome, abnormal LFTs.
Drug Interactions
Possible interaction w/ oral anticoagulants (eg, warfarin, dabigatran), ASA, & other NSAIDs (eg, ibuprofen); other centrally-acting drugs or substances, including alcohol & sedative drugs (benzodiazepines, opiates, antipsychotics, phenobarb, sedative antihistamines). Serotonin syndrome or NMS-like reactions w/ triptans; tricyclics, lithium, SSRIs, SNRIs, or dopamine antagonists, including antipsychotics; amphetamines; sibutramine; fentanyl, tramadol, tapentadol, meperidine, methadone, pentazocine; dextromethorphan; St. John's wort; MAOIs (including linezolid & methylene blue); tryptophan supplements; venlafaxine or other medicines containing desvenlafaxine. Avoid alcohol intake during therapy.
